2026-2027 LLCA Required Parent Registration and Permissions for All Students

The information in this form must be completed annually for each current LLCA student. 

 
Lake Lure Classical Academy is required by the state and federal government to collect much of the personal information in this form. All information is confidential and is not shared with anyone. 
 
In order for our school to qualify for Title 1 funds,  it is very important for us to get the economic data requested in the last section of this form. Additionally, this is how students qualify for free or reduced lunch through our school food truck. We understand that a small percentage of parents are reluctant to disclose financial information on a document or form. Because of this, we have not marked section 7 as "required" information. However, if you choose not to complete section 7, please call the office and speak to the Data Manager to let us know if your student would qualify as economically disadvantaged.

Media and Photographic Release Information

Occasionally, we will have an opportunity for photos of Lake Lure Classical Academy students to be featured in news articles, news releases, school newsletters, school websites, social media, or DoJo (for elementary students). We love to share the exciting happenings at LLCA.

Transportation Information

LLCA transports students to and from athletic games, practices, field trips and off-site activities during the school year. This form serves as permission for transportation. You will be informed prior to your student traveling by bus.

Policy Agreement

I am aware that Lake Lure Classical Academy has code of conduct guidelines: dress codes, academic standards, attendance policy, Chromebook agreements, etc. All of these can be found on the website at llcharter.org in the Student / Parent Portal under the Student Handbook button. Compliance with these standards is implicit upon registration at Lake Lure Classical Academy.

North Carolina has recently introduced restrictions on student cell phone use in public schools. Effective January 1, 2026, House Bill 959, signed into law by Governor Josh Stein, prohibits student cellphone use during instructional time, with exceptions granted by teachers. Because of this new bill, LLCA is adding additional policies concerning the use of cell phones. You will be notified when the student handbook has been updated to include these policies.

Student Medical Services

Knowing that LLCA personnel will take utmost care of my student's safety, I understand that accidents do occur and that in such a situation, immediate steps must be taken to secure my student's health.

I authorize LLCA representatives to seek medical attention for my student should an emergency arise, provided that I will be contacted as soon as possible. I further agree that LLCA or their representative will be held harmless in the event of accident or injury. I understand that LLCA disclaims any and all liability in the unlikely event of injuries sustained at any LLCA sponsored activity or event.

2026-2027 Household Income Reporting (required for Title I funding)

We are required by the State of North Carolina to report test results for the following student groups: ethnic origin, students with disabilities, English language learners, and students identified as economically disadvantaged. To report on the economically disadvantaged subgroup, household income information must be collected. All information in this section is confidential and maintained by office personnel only. 

 
Students attending public schools including charter schools may be eligible for supplemental education services, resources, and opportunities based on the size and income of the household in which they live. Please complete this form to help your child's school determine whether your child(ren) may qualify for additional educational support.
